The Information Network for Oregon Hospitals (INFOH) is the state-mandated discharge data reporting program that collects data from all Oregon hospitals and three border hospitals in Vancouver, WA. Hospitals submit inpatient and outpatient claims data quarterly, which allows hospitals to participate in quality performance initiatives and other analyses.

Apprise Health Insights works with the Oregon Health Authority (OHA) to help hospitals continue to drive health system transformation and improve patient quality. Hospitals participating in HTPP work to improve upon eleven quality measures and are funded based upon their performance.

In 2012, Apprise worked with 31 hospitals in the state of Oregon, the American Hospital Association, and other healthcare community partners to implement the CMS initiative, Partnership for Patients. During the 2 year stint of the program, participating Oregon hospitals collectively lowered readmissions by 20% and patient harm by 40%!

Starting in 2015, the Partnership for Patients work is in its second iteration with a brand new way for hospitals to submit data utilizing a browser-based data reporting platform.
